Position Title: Resettlement Case Manager

Reports to (Position): Resettlement Supervisor

Essential Functions:

  • Provides case management services to refugees and other displaced persons to include coordination of housing, orientation/client education, and other basic needs enrollments within program requirements and time frames utilizing in-house services and referrals to external service providers within the community
  • Assists the client to engage in activities that will facilitate the clients self-sufficiency and ability to perform tasks independently
  • Coordinates the provision of interpretation and translation services to clients
  • Provides safe and supervised transport for clients to offsite appointments or activities as needed and confirms public transport capabilities.
  • Completes all case notes and program documentation, ensures accuracy and compliance within program guidelines (core services of the Cooperative Agreement) and confirms timely delivery of services
  • Enters data in the agency database in a timely manner as directed
  • Communicates, coordinates and promotes program within community and partners.
  • Serve as an advocate for clients
  • Perform other duties as assigned
  • Adheres to policies related to boundaries with youth.
  • Attends required abuse risk management training.
  • Adheres to procedures related to managing high-risk activities and supervising youth.
  • Reports suspicious or inappropriate behaviors and policy violations.
  • Follows mandated abuse reporting requirements.

Education: (Include degrees or diplomas required or equivalent experience.)

  • Graduate of an accredited 4 year college or university with major course work in Social Work, Sociology, Psychology or a related field
  • A comparable amount of training and experience may be substituted for degree

Experience: (List the nature and length of experience required.)

  • Background in social services required
  • Previous case management experience 1-3 years preferred; Experience working with diverse populations and refugee social services programs preferred
  • Experience and education may be substituted for one another.

Knowledge/Skills: (List knowledge, skills, and abilities required.)

  • High English language proficiency and excellent communication skills both verbal and written required
  • Computer proficient including Microsoft Office applications (Word, Excel and Outlook) required
  • Fluency in an additional language spoken by the client base preferred

Licenses/Certifications: (List licenses or certifications required.)

Valid Texas drivers license and minimum mandatory Texas automobile insurance coverage; Must be able to provide own daily transportation

Physical Requirements and Working Conditions:

Regular work hours are 8:30am-4:30pm. Work hours may occasionally be required before or after daylight hours. Considerable time will be spent escorting clients to appointments and meetings at various locations. Work may be stressful when dealing with individuals and/or working within time constraints.

Work requires some physical activity including frequent sitting, standing, walking and reaching, and occasional bending, kneeling, crouching and stooping.

Work requires the ability to frequently lift and/or carry equipment, supplies and other materials weighing up to 10 pounds, and occasionally lift and/or carry equipment, supplies and other materials weighing up to 30 pounds.

Work requires sufficient hand-eye coordination and manual dexterity to use hands and arms to reach, handle, grasp and feel, and to operate computers and other office equipment.

Work requires sensory abilities including vision correctable to normal ranges. Work requires talking and hearing within normal ranges and ability to communicate accurate, concise information. Noise level in the work environment is usually quiet to moderate. Office work may be subject to frequent interruptions.

May be exposed to communicable diseases.
