Description

EAC Network is seeking a Bilingual Forensic Case Manager for the Manhattan Health Court Evaluation Services program, located in Brooklyn, NY. This full-time position operates Monday through Friday from 9:00 AM to 5:00 PM and offers excellent benefits, including medical, dental, 401K, and generous Paid Time Off (PTO).

The Forensic Case Manager will provide essential services, including identification, screening, treatment planning, case management, and court reporting for eligible participants living with mental illness or co-occurring disorders.

Principal Duties & Responsibilities

  • Act as a liaison to the courts and correctional facilities.
  • Provide case-finding and screening services within the Court to develop appropriate service plans for participants.
  • Implement comprehensive screenings to facilitate referrals to treatment and arrange transportation for clients when necessary.
  • Submit timely monitoring reports to the MMHC Resource Coordinator before each scheduled court appearance, detailing participants’ compliance with court-approved treatment plans.
  • Notify the court immediately if participants commit major infractions.
  • Maintain case documentation for supervision review, including client records and progress notes.
  • Assist clients in applying for and receiving public and private benefits.
  • Manage a caseload and monitor client participation in the treatment program through case management appointments and progress report reviews.
  • Conduct periodic visits to treatment facilities and maintain an accurate log of visits.
  • Prepare and present written and oral status reports to the criminal justice system regarding client progress.
  • Ensure appropriate levels of contact with program participants and treatment providers.
  • Participate in case conferences weekly or as needed.
  • Investigate and arrange supportive services and entitlements for clients.
  • Attend relevant training or continuing education opportunities as directed by the Program Director.
  • Conduct supervised toxicology on participants as needed.
  • Serve as a resource for staff members regarding treatment planning and needs.
  • Assist the Program Director with administrative tasks related to program management.

Knowledge, Skills, & Abilities Required

  • Bachelor’s degree required; Master’s degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology preferred, along with one or more years of experience working with the criminal justice, substance-abusing, or mentally ill population.
  • Must be bilingual fluent in Spanish.
  • Strong communication skills, both oral and written, with the ability to work cooperatively with treatment and criminal justice systems.
  • Proficiency in computer applications or automated systems, including spreadsheets, Microsoft Office applications, calendar, email, and database software necessary for work assignments.
