Job Overview

EAC Network is actively seeking a dedicated Bilingual Case Manager (English/Spanish) to join our Nassau Adult Guardianship program in Nassau County. This full-time role operates Monday through Friday from 9:00 AM to 5:00 PM, with potential flexibility for evenings and weekends. Key Responsibilities

  • Initiate contact with relevant stakeholders within 72 business hours of case assignment to commence the fact-finding process and develop a tailored service plan for Guardianship clients.
  • Deliver case management services, including home visits, telephone reassurance, and advocacy for clients facing challenges such as substance abuse, mental health issues, housing instability, and financial difficulties.
  • Facilitate client transportation to appointments as necessary, utilizing personal vehicles for emergency resource access or application completion.
  • Engage in case conferencing for thorough reviews and informed decision-making.
  • Maintain accurate and up-to-date progress notes for all assigned cases.
  • Communicate effectively with judges, legal representatives, and DSS caseworkers regarding program participants, while also presenting cases to community referral sources.
  • Complete and submit Initial, Interim, and Annual Reports, as well as court updates for clients.
  • Provide necessary progress notes to the finance department for billing purposes.
  • Develop evaluation strategies to assess program effectiveness.

Required Skills

  • Bachelor’s Degree in social or behavioral sciences is required.
  • Proficiency in office applications and computer systems, including Microsoft Office and database software, is essential for fulfilling job duties.
  • Bilingual proficiency (English/Spanish) is mandatory.
  • A minimum of two years of relevant experience is preferred.
  • Strong interpersonal and communication skills, both verbal and written, are crucial.
  • Ability to motivate and support clients effectively.
  • Valid NY State Driver’s license and access to a personal vehicle are required.

Qualifications

  • A Bachelor’s Degree in social or behavioral sciences is essential.
  • At least two years of related work experience is preferred to ensure familiarity with case management practices.
