Job Title: Social Worker Site Supervisor

Department: Tenant and Community Services

Employment Status: Full-Time;Exempt

Pay Range: $82,700 – $87,300/year

Location: 44 McAllister St, San Francisco, CA 94102

Schedule: 4 days onsite, 1 day remote – to be negotiated with Supervisor

8:30am-5:00pm on Monday, other days can be 8:30-5pm, 9am-5:30pm

Summary

Under the general direction of the Support Services Manager, the Social Worker Site Supervisor is responsible for assisting residents living in TNDC properties with housing stabilization & retention by providing counseling and support services, community outreach, advocacy, information, and referrals to existing off-site providers. The Social Worker Site Supervisor is also responsible for the supervision of off-site Social Workers.

Essential Duties:

  • Deliver support services programs for tenants with a lens of cultural humility
  • Supervise on and off-site Social Work Staff, including hiring, orienting, and training new staff.
  • Manage a caseload of individuals, conducting comprehensive intakes and assessments.
  • Regularly outreach to assigned tenants, including home visits, to identify and address their needs, such as medical, mental health, and substance use.
  • Provide trauma-informed case management, supportive counseling, crisis intervention, and conflict resolution.
  • Provide housing stabilization, retention and eviction prevention services.
  • Provide tenants with family focused information and referrals to off-site service providers in the community.
  • Provide supportive counseling, and case management services to ensure the successful transition of clients and their families to community-based resources.
  • Provide crisis intervention, conflict resolution.
  • Assist tenants to develop positive exit plans when appropriate and provide tenants with up to 30-days of after-care
  • Organize and facilitate community-building activities, including produce drops, cultural celebrations, social events, workshops, and informational sessions.
  • Serve as a liaison/advocate for tenants with Property Management and off-site service providers, attending weekly meetings with assigned Property Management staff and regularly scheduled meetings.
  • Develop relationships with outside service providers and attend monthly off-site community meetings as assigned.
  • Comply with all mandated reporting requirements, including Adult Protective Services & Child Protective Services
  • Ensure compliance with all mandated reporting requirements, HIPAA, PHI policies and procedures, and maintain accurate records using TNDC’s CIRCE electronic database and SF City and County Clarity ONE System.
  • Participate in processing all intakes for targeted DPH, MHSA, and S+C referrals through San Francisco’s Coordinated Entry System in partnership with TNDC’s Property Management.
  • Carry and promptly respond to cell phone calls daily and after regular business hours.
  • Perform other duties as assigned.

Minimum Qualifications:

  • Bachelor’s degree combined with 3-5 years of relevant professional experience.
  • Bi-lingual: Spanish/English
  • Two years of experience working with individuals with low-income in a social service setting.
  • Two years of experience working with families and/or elderly populations
  • One year of experience providing supervision and oversight to staff
  • Two years of experience providing direct services to individuals with substance use, mental health issues, and/or co-occurring disorders, including linking them to necessary agencies.
  • DOJ Livescan upon hire
  • TB test upon hire or proof of TB test within the year

Preferred Qualifications:

  • Masters Degree in Social Work or a related field.
  • Experience working in a Permanent Supportive Housing setting.
  • Experience providing housing stabilization, housing retention, and eviction prevention services
  • Knowledge of current resources and service/benefits programs in San Francisco and the Greater Bay Area.
